Family arguments about care rarely start with bad intent. They start with a missing record. Was the morning dose given? When did the fall happen, and who was told? One person remembers it one way, another remembers it differently, and memory argues. A shared record does not. It simply shows what was written down, on the day it happened, by the person who was there.
The Elder Care Daily Medication & Incident Log is a calm, large-type 10-page log for families and aides: every dose checked off as given, missed, or refused, every incident recorded the same day with who was told and what follow-up happened, and a weekly family summary that puts the same facts in front of everyone. This is a logging and organization tool only. It gives no medical advice and no dosage guidance.
What it is
This is a printable log, a fillable phone version, and a tracker spreadsheet that work together as one shared care record:
- START HERE — where your files live in your Etsy account, how to open them, and which file is for what
- Care Profile — the person, doctors, pharmacy, and emergency contacts — never a Social Security number, passwords, or full account numbers
- Medication Master List — name, dose, time, purpose, and prescriber, copied exactly from the label
- Daily Medication Checkoff Sheet — date, dose given / missed / refused, plus initials, one sheet per day
- Incident Log — falls, confusion, refusal, injury: what happened, when, who was told, follow-up
- Vitals & Wellbeing Daily Notes — readings as observed, recorded not interpreted
- Appetite, Hydration & Sleep Checkoff — the quiet daily signals, checked off every day
- Aide Shift Handover Notes — what the next aide needs to know, signed
- Doctor & Pharmacy Call Log — who you spoke to and what they actually said
- Weekly Family Summary — share the record, end the dispute
The fillable PDF version is included — open it on your phone and tap-type into 204 fields. No printing is required to complete it, and the large type keeps the pages easy to read for everyone using them.
The tracker spreadsheet (.xlsx, for Excel and Google Sheets) does the counting: a Medication Tracker that counts doses given, missed, and refused automatically, an Incident Log that counts by type, and a Weekly Summary that pulls both together — with demo data so you can see it working in about a minute.
Why it was created
Care is often shared — a daughter on weekdays, a son on weekends, an aide in between — and shared care fails at the handover. A dose given at breakfast is invisible to the person who arrives at dinner unless someone wrote it down. A fall that seemed minor on Tuesday is forgotten by the doctor’s appointment on Friday. This log was created to give every caregiver, family or paid, the same simple habit: check off the dose when it happens, record the incident the same day, hand over the shift in writing, and roll it all into one weekly summary the whole family can read.
Who it’s for
- Adult children sharing care for a parent and tired of comparing memories
- Family caregivers who need a clear daily record for doctors and pharmacies
- Home aides and the families who employ them, who need a signed handover between shifts
- Anyone caring for an older person whose medications and incidents must be tracked carefully, every day
How to use it
Begin with the Care Profile and the Medication Master List, copying each medication exactly from its label — name, dose, time, purpose, and prescriber — and never changing a dose or schedule in the log on your own. Use one Daily Medication Checkoff Sheet per day and mark each dose given, missed, or refused at the time, with initials. Record any fall, confusion, refusal, or injury in the Incident Log the same day: what happened, when, who was told, and what follow-up was done. Note vitals and wellbeing as observed, check appetite, hydration, and sleep daily, and close each shift with signed Handover Notes so the next person starts informed. Log doctor and pharmacy calls with what was actually said, then share the Weekly Family Summary so everyone works from the same page.
This log is a record-keeping tool only. It is not medical advice and gives no dosage guidance. Always follow the prescriber’s instructions on the medication label and care plan, contact a qualified health professional with any health question, and call emergency services for emergencies. It is not legal advice, consult an attorney about your situation if a legal question arises around care or records.
