Creole Intake — Haitian Creole Voice Intake is a front-end client intake form that speaks Kreyòl first. Drop the [creole_intake] shortcode on any page. Every label is Haitian Creole with a small English subtitle. Clients enter their name, phone, optional email, category of need (Imigrasyon / Taks / Lojman / Travay / Fanmi / Lòt), preferred contact method, and a message — or skip typing entirely and record a voice note right in the browser.

What you get

• Voice notes, no typing required. Record, preview, and re-record in the browser (3-minute cap with on-screen timer). The audio rides along with the submission — no apps, no accounts, no transcription fees.
• Admin inbox. New requests arrive in wp-admin with an unread-count badge, New/Read filters, click-to-call phone numbers, and an in-browser audio player for every voice note. You get an email on every new request.
• Phrase Helper. 30 printable Kreyòl → English front-desk phrases (greetings, client info, documents & appointments, reassurance & closing) — print it and keep it at the front desk for callbacks.
• Your server, your data. The voice recorder is the browser’s built-in MediaRecorder — no external services, no per-minute fees. The client’s audio stays on your own server, never shipped to a third party.
• Built for a nervous person on a phone. Plain language, high contrast, large tap targets. No microphone or no HTTPS? The voice section hides itself with a Kreyòl note and the text form still works.

Spam-resistant out of the box: honeypot field plus per-IP rate limiting. Single-file plugin, zero dependencies, wp-admin native.
